then Bridging The Gap Care Management Services may be able to help improve the overall health and well-being of the individual. Bridging The Gap Care Management Services can provide support and answers to help you or your child enjoy the benefits of fully coordinated and collaborative care. These services offer a variety of benefits including:

  • Providing effective coordination and collaboration between all services needed.
  • Helping children or adults with chronic conditions receive the right mix of preventive services.
  • Improving the overall health of eligible participants.
  • Decreasing the need for acute-care interventions.
  • Controlling healthcare costs.

WHAT IS A HEALTH HOME

A Health Home is not an actual tangible location. Rather, it is a PROCESS in which all medical, behavioral health and/or social services are coordinated through a single source. Some of these services might include:

  • Connecting to health care, mental health and/or substance abuse providers.
  • Connecting to needed medications.
  • Help with housing.
  • Social services (such as food, benefits, and transportation).
  • Other community programs that can support and assist you or your child.

WHAT IS CARE MANAGEMENT

Care Management is the coordination of all services (behavioral health, medical and social services, and more) for the enrolled adult or child. By coordinating services, it is easier for clients with chronic conditions to receive the right preventative services, at the right time and in the right amount. This improving their overall health, decreasing the need for acute-care interventions, and controlling healthcare costs.

 

How care management works

  1. After an individual, family, provider or school contacts Bridging The Gap Management Services, LLC, we will help determine whether the adult or child is eligible.
  2. If eligible, they are assigned a Bridging The Gap Care Manager who will meet regularly with the individual or family to assess their ongoing needs and review their current services.
  3. The Care Manager follows the progress of the client’s care, and coordinates between the individual/family and other providers to ensure the individual is receiving the best possible combination of services.
